Before your appointment

When forms are assigned to your visit type, Charm sends them to your patient portal and through secure email/SMS links. Open the practice's message and complete the assigned questionnaires and consent forms before your visit. If you cannot find them or need an invitation, call the practice for help.

Detailed intake is handled in Charm. Please do not send completed medical questionnaires through ordinary email.
